平素より当院をご利用いただき、誠にありがとうございます。
当院では臨時休診後、今後の診療継続について検討してまいりましたが、
医師の勤務再開の見通しが立たず、診療を継続することが困難であると判断いたしました。
そのため、当院は2026年9月30日をもちまして閉院いたします。
現在、医師が勤務できないため、
診療、処方箋の発行ならびに診断書・傷病手当金申請書等、医師による作成または証明が必要な各種書類には対応できません。
継続的な診療や処方が必要な患者様におかれましては、お早めに他の医療機関へご相談くださいますようお願いいたします。
閉院後の各種手続きおよびお問い合わせ方法につきましては、現在調整を進めております。
詳細が決まり次第、当院ホームページにて改めてご案内いたします。
なお、当院の公式LINEは閉院に伴い終了を予定しております。

感謝各位患者一直以來對本院的信任與支持。
自本院暫時休診以來，我們持續評估恢復診療的可能性。
然而，由於目前仍無法確定醫師何時能夠恢復執業，本院判斷已難以繼續提供診療服務。
因此，本院將於2026年9月30日正式閉院。
對於公告時間較晚，以及突如其來的消息，我們深感抱歉。
由於醫師目前無法執業，本院現階段無法提供診療及開立處方箋，
也無法製作或簽署診斷書、傷病手當金申請書等需要醫師填寫或證明的各類文件。
如您仍有持續就醫或用藥的需求，敬請儘早洽詢其他醫療院所。
關於閉院後的各項手續及聯絡方式，目前仍在進行協調與確認。詳細資訊確定後，將另行於本院網站公告。

【Notice of Clinic Closure】
Thank you for your continued trust in and support of our clinic.
Since the temporary suspension of our services, we have carefully considered whether it would be possible to resume medical care. However, as there is currently no foreseeable date on which the doctor will be able to return to work, we have concluded that it is no longer possible for the clinic to continue providing medical services.
Therefore, our clinic will permanently close on September 30, 2026.
We sincerely apologize for the delay in making this announcement and for the sudden nature of this notice.
As the doctor is currently unable to practice, we are unable to provide medical consultations or issue prescriptions. We are also unable to prepare or certify documents requiring a doctor’s assessment or signature, including medical certificates and Sickness and Injury Allowance application forms.
Patients who require continued medical care or prescriptions are kindly requested to consult another medical institution as soon as possible.
Arrangements concerning administrative procedures and methods of contact following the closure are currently being finalized. Further information will be announced on our website once the details have been confirmed.
Please note that our official LINE account is scheduled to be discontinued following the clinic’s closure.
